Did your wife receive a Form DS-3032 from the NVC? They used to send that form directly to the applicant (your wife).
If she did not receive it, but it was sent to you instead, then it should technically be signed and dated by your wife before you return it. I’m not sure how strict they are on the signatures, but the best bet is to send it with her original signature. I know some lawyers send them in without applicant signatures, but I’m not sure if you can get away with it or not.
Does the DS-3032 that they sent you have a barcode on it? The NVC can get pretty picky about using the form with a barcode when you return it so they can process it electronically. If you have to send her the DSD-3032 with barcode, I would complete the form and then send it to her via FedEx or other rapid, safe means for her to sign, date, and return to you. I would not send via FAX.
If there is no barcode on the Form DS-3032 they sent you and she didn’t receive one from NVC either, then she can download a form and sign it and date it, then send it back to you so you can forward it to the NVC.

There is no fee for the DS-3032 and it does not need to be notarized.
