It was initially required of males from Muslim countries and then later required from most all males between the ages of 16-45 applying for NI visas to the U.S.
The Manila embassy doesn’t usually require this for females, but if they sent her one then she should complete it and turn it in with the rest of her package. Just have her complete it the best she can, making sure that all the personal data correlates with that on the other forms she completed (DS-156, G-325A, etc.)

Perhaps they want her to complete a DS-157 because she comes from a Muslim region of Mindanao???
For the DS-156, she can Xerox the original on plain white paper, but the signatures/dates should be original on both copies.
