Question for the board,

My wife in her K3 package said there is a DS 157
What is that for and does it pertain to us?
It is a SUPPLEMENTARY NONIMMIGRANT VISA APPLICATION what do we do with that?

I sent her a copy of the DS 156 all filled in by type writer with only her passport information missing,  but only sent one copy.  Can she make another copy or should she just start over and use the two copies that was sent to her in the K3 package?  We don’t want to go to the bank and pay that $100 for the stamp to show the visa payment till that is decided.

Thanks

The DS-157 was implemented after 9/11 as an additional security tool to help screen out terrorists, particularly Muslim terrorists. It was initially required of males from Muslim countries and then later required from most all males between the ages of 16-45 applying for NI visas to the U.S.

The Manila embassy doesn’t usually require this for females, but if they sent her one then she should complete it and turn it in with the rest of her package. Just have her complete it the best she can, making sure that all the personal data correlates with that on the other forms she completed (DS-156, G-325A, etc.)

There is a fillable version at the State Dept Web site:

http://travel.state.gov/DS-0157.pdf

Perhaps they want her to complete a DS-157 because she comes from a Muslim region of Mindanao???

For the DS-156, she can Xerox the original on plain white paper, but the signatures/dates should be original on both copies.
