Your fiancée is correct. The locally issued birth certificates are acceptable for most all situations but the U.S. Consulate requires that they be issued by the NSO in Manila on special security paper for visa purposes. She can not get those in Cebu.
If she goes to Manila to request it in person, it usually takes 1-2 weeks to get pick them up. She can also have a friend or relative in Manila do it in person for her with written authorization. Like some other posters already mentioned, the NSO now has procedures to request documents by phone or on line. You can read more info here:

My wife ordered documents by phone and it took a long time to get through. It takes a lot of patience and you just have to keep dialing the number. They are available 24 hours and it’s usually best to call late at night (1-3 a.m.). If she is using a cell phone, she should have it loaded with maximum minutes and it's a toll call from Cebu. When she gets through, an assistant will explain everything to her and tell her how to make the fee payment. She should have a copy of her birth certificate handy when she calls.
I think the best bet would be the new “E-Census” on line system.
