Approx $150 to cover the cost of the visa application fee and the medical exam.
Passport valid for at least one year.
2 copies of the OF-156 Visa Application.
DS-156K Fiancée Visa Application.
Certified copy of birth certificate from NSO Manila for fiancée & any children.
NBI clearance and police clearance from any country where she lived for more than a year.
I-134 Affidavit of Support with all supporting documents.
Evidence of meeting (couple of photos & copy of passport).
Evidence of relationship (letters, phone bills, receipts, etc.)
3-4 visa photos.
Complete copy of any divorce decrees.
A recent copy of her baptismal certificate if available, especially if her birth was late-registered.
Originals of any document copies you submitted with the petition.
Some of the readers with recent experience can correct anything I missed.
