Question #10 asks for my Fiancee's  National Identification Number...what is this?? I thought it was like our Social Security number,but she says she is not sure. Any help appreciated.

I'm about ready to send my Affidavit of Support over. I got my letter from work. Should my bank statement be signed by officer of the bank? Is a teller alright? Also,is it o.k. to send copies of my W-2's  or should i send the originals? I have my 1040's from last 3 years,should they be notorized?...Any help or advise appreciated as always. One more thing,if paperwork is in order will she be able to skip interview? She is shy and a little nervous about it.

Red,

Send copies and include the Original Documents Form FC-029, to verify that all photocopies are true copies of originals.  A bank officer should sign the letter which verifies the date the account was opened.  The bank should also be able to notarize the 1040 copies while you're there, first page of each year.

Hi Red,

I sent copies of my last 3 years 1040's and W-2's. I didn't notarize mine and had no problems. I did include a notarized copy of Form FC-029.

which states about the I-134:

"IF you qualify on income alone, you do NOT have to submit the evidence of other assets such as bank records/letter."

What they want is not a monthly bank statement, but a statement signed by a bank employee (preferably the one who wrote the letter) on bank letterhead, stating the date the account was opened, total deposits for the past 12 months and the present balance. I believe in covering all of the bases, so I included the bank statement.

like mine, a very large Bank system (Key Bank) does not have the record keeping ability to tally a years total deposits, but can only record account activity on a month to month cycle. I had to opt for 12 statements.

Hi SJ,

That sounds like a reasonable alternative. Great Western Bank had my average balance per month for the past 12 months, but had to add my deposits by hand. The officer had never done it before and was trying to find a program that would add them up, to no avail. Fortunately the lady was very nice, especially after I explained the reason that I needed it. She came to the US on a Spousal Visa from Colombia. I doubt that many others would have taken the time required to do it. Including printer failure, it took about an hour, as I have a very active account. I had been certain that I would have to talk with the branch manager or above.

Red, question number #10...the answer is to write "none" in that  box. (This assumes your girl is from the Philippines, there is no National identification number there).
I never sent a bank statement with my Affadavit of Support, they want a printout of all deposits and withdrawels for the last 12 months signed by an "officer " of the bank , not just your last statement signed by a teller. If you are earning over the amount required to petition (around 23K a year I think) , then you could do a I did and send your last 3 years tax RETURNS (copies) and W-2's (copies) and have them NOTARIZED. I think for an Spousal they want 3 years and a fiance they only need one year but I sent 3 years just in case. And there are no more interviews in the Philippines if everything is in order, on her second day at St. Lukes they will take the paperwork and then you will wait for courior to deliver the visa.
Jicelle finished on May 7th, and they told her about 14 working days. We are hoping this week she will receive her visa! By the way the people at St. Lukes must have gotten some heat, because they took all the paperwork and documentation, EVERYTHING!
