Although my wife has not been yet back in Ukraine since our marriage two years ago, we have enough Ukrainian friends with personal experience with PMZh stamp (for Ukrainian citizens with permanent residency abroad). I understand that this stamp is definitely required. I was told of cases when people were returned from passport control at the departure resulting in delaying the trip back home for 1-3 weeks. However, I also heard that these troubles can be prevented eg by travelling on "Ukrainian passport" (sort of an ID document in Ukraine) to Russia and then from Russia on "international" Ukrainian passport (actual travel document).
Maybe the best way is to arrange the PMZh stamp prior permanent departure from Ukraine. If already living abroad, it may be arranged from via Ukrainian Embassy - this may however take some time (several weeks to months - depends on efficiency of local Embassy...)
PS My wife does not have the exit stamp either so this would be our concern too if travelling to Ukraine. I think in our case we would arrange our trip via Russia....
