Ray, Ray, Ray....co'm man, cut the dude some slack. Two whole MONTHS ago? He hasn't just forgotten that he asked the question, he's probably forgotten whom he intended to list as the beneficiary. Be that as it may, who ever wants to can download the forms and read all the requirements right here: http://www.uscis.gov/files/form/I-129F.pdf
of course the directions in that form will refer you to other forms you also have to file, but would anybody care to hazard a guess where those forms can be found?
